Administration of vitamins and low-dose hydrocortisone in the supportive therapy of septic patients

Description of the research topic:

Sepsis is a complex syndrome, an oversized response of the body to an invasive infection. Mortality from sepsis and septic shock is still high nowdays. Numerous studies have demonstrated the mutually reinforcing positive effects of vitamin C and hydrocortisone in this condition, but it isn’t included in the current international recommendation yet.
The effects of drugcombination are the following:
• inhibits NF-κB activity, regulates the release of proinflammatory cytokines
• increases the number of tight junctions between endothelial and epithelial cells
• helps maintain endothelial function and microcircular flow integrity
• required for catecholamine synthesis and increase vasopressor sensitivity.
High doses of intravenous vitamin C increases the metabolic conversion of oxalate, glyoxylate aminotransferase reduces the conversion to oxalate, and the cofactor of the enzyme is thiamine pyrophosphate. Therefore, intravenous thiamine supplementation is required.
Our aim is to investigate how the concomitant use of vitamin C, hydrocortisone and thiamine affects the length of mechanical ventilation, vasopressor days, kidney failure requiring of renal replacement therapy, length of stay in the ICU and mortality compared to the control group in patients with severe bacterial and viral (SARS COVID-19) septic disease.
A retrospective and prospective clinical study.
