Quality of life measurement and its implication in developing modern therapeutic strategies in non ST elevation myocardial infarction

Cardiovascular disease is the leading cause of death in western type, industrialized countries. Among these, acute coronary syndromes are the most prevalent manifestations. Non ST elevation myocardial infarction (NSTEMI) is twice as frequent as the more recognized ST elevation myocardial infarction(STEMI) . Based on large randomized clinical trials and metaanalyses, invasive approach, i.e. percutaneus coronary intervention (PCI) is recommended as the first therapeutic option. Coronarography frequently depicts multiplex coronary disease. Coronary intervention on non culprit arteries during primary PCI is controversial. Measurement of fractional flow reserve (FFR) is an emerging tool in the hands of the interventionalist. Fractional flow reserve considered to identify true ischemic lasions. We decided to evaluate the significance of FFR measeurenment in NSTEMI patients during primary PCI. We randomize two groups of patients. In the conservative arm in our study we treat the culprit lesion only, in the more invasive group we treat the culprit lesion and the non culprit true ischemic lesions measured by FFR. The primary endpoint is the major coronary adverse events (MACE). Additionally we decide to identify the quality of life (QOL) of both groups using the generally accepted WHOQOL-BREF quationnaire
