Thyroid Imaging Reporting and Data System (TIRADS) is the basis of the management of nodular goiter patients. The characteristics used for generating TIRADS are those which help us to recognize the most frequent thyroid cancer type, the papillary cancer. Although there are thousands of papers have been published about the system, some basic issues still remain to be cleared. E.g. neither the interobserver, nor the intraobserver variability in the judgement of these features have been tested in real-world circumstances, the basis of all studies are selected still images. It is also not clear which is our primary goal, i.e. which cancers are the targets of the evaluation. Although TIRADS seems to be useful for recognition of papillary cancer, clinicians use the system for all kinds of cancers. It has not been established whether the system is useful for recognizing non-papillary thyroid cancers. The goal of the program is to analyze these problems, and therefore to get closer to the real advantages and disadvantages of the TIRADS, and more broadly, to reveal the real benefits and pitfalls of the decision on aspiration cytology based on selected ultrasound features.
