Saját és mások munkái alapján a fibroblast bizonyult az EOP-t kiváltó és fenntartó tényezők fő támadáspontjának. A pathomechanizmus részletei nem ismertek. Lokális citokinek, parakrin tényezők hatását vizsgáljuk a fibroblast proliferatiora és matrix-szintézisre.
